Order form for 2008
Modifications can be made to the order form to suit the needs of your
particular group.
This form requires that the initial purchase includes an original and
one extra copy. If you prefer to offer
the original without a mandatory
extra copy, that is fine.
You may prefer to offer only mats
and not the oval frames. To simplify the fundraiser, only individually mounted/ matted silhouettes may be offered as well. For a larger view
